For private purpose and functionality. it´s necessary allow add more custom fields for example NIF (personal identification), and menu of option (combo box), description of problem for the appointment.
Is it posible add the fields right now? do you have an ETA to add it?

Please try to add attributes in Bookings/Attributes and please enable Allow front-end editing option for each attribute that you want to add to make reservations form
