Add fields in vendors registration form

Hi, is there a way to make switcher Private person/Company to the vendors registration form. And if person choose Private persone than First and Last name fields to be displayed and if person choose Company than Company name and Organisation number fields to be shown?

And if you can not provide me with code that solves the problem above can you give me snippet that adds telephon number into vendors registration form? And this field to be placed bellow Last name field.
